crew visa requirements

 

This visa is issued to people arriving as crew on international planes, ships or trains.

 

Passport

 

You must provide your actual signed passport, including one copy of the personal information page of your passport. Your passport must:

 

  • The passport must have at least 12 months validity remaining before expiration.
  • Contain at least two blank visa pages for the visa stamp (amendments and endorsement pages cannot be used to fulfill this requirement).

 

Photograph

 

You must provide one 2″x 2″ passport type photo.

 

Business Letter
 

Letter from the applicant's employer (i.e airline) or sponsoring company on company letterhead.

 

Crew Member ID

 

A a copy of your crew member ID.

 
Proof of State Residency

 

Traveler's will be expected to submit a copy of their valid Driver's License to prove their address. Minors must submit a parents Driver's License along with a copy of the birth certificate.

 

For Non-U. S. Citizens:

 

You must send a photo copy of your U.S. Green Card or your valid U.S. visa.

3 business days processing,    consulate fee $170, service fee $199

4-5 business days processing, consulate fee $160, service fee $139

6-8 business days processing, consulate fee $140, service fee $89
